A community care product platform was developed to help disability and aged care providers simplify complex onboarding, agreement management, forecasting and claims generation. The platform brought key care operations into a more connected digital environment, giving providers a clearer way to manage service information, funding visibility and high-volume claims activity.

Through a multi-year delivery partnership, iSOFT supported product engineering, implementation, third-party integration and ongoing enhancement across multiple provider environments. This helped translate complex care operations into a scalable product platform that could continue evolving with provider needs.

The Challenge

Disability and aged care providers were managing complex onboarding, service agreements, budgets and claims through fragmented processes. This created delays across care administration, limited visibility of available funding and made it harder for teams to deliver a consistent customer experience across multiple provider environments.

As service delivery became more complex, providers needed a more reliable way to connect onboarding, agreement management, forecasting and claims generation. Manual processes made it difficult to manage available resources efficiently, reduce rework and keep high-volume claims activity moving with the speed and accuracy required. The platform needed to reduce manual effort, improve forecasting accuracy and give teams a clearer view of the information required to manage care delivery with confidence.

Our Approach

iSOFT provided end-to-end product delivery support across functional and technical roles, helping translate complex community care operations into a scalable digital platform. The work brought together product engineering, implementation, solution architecture, development, integration and ongoing support, ensuring the platform could operate reliably across multiple provider environments.

The solution was delivered on the Microsoft stack, using Dynamics 365, Power Platform, Power Automate, Power Pages and Azure services, including Azure Functions and Logic Apps. This provided a flexible foundation for onboarding, agreement management, forecasting, claims generation and operational reporting, while supporting the broader need for accuracy, visibility and repeatable service delivery.

iSOFT also supported third-party integration and ongoing enhancement, helping the product continue evolving with provider needs. Rather than delivering a one-off implementation, the engagement focused on building a maintainable platform that could be improved, scaled and supported over time as care operations and provider requirements continued to change.

The Outcome

The platform delivered measurable improvements across core community care operations, including a 25 to 30 percent reduction in onboarding time, a 15 to 30 percent increase in forecasting accuracy, zero claim rejections, and more than 60,000 claims generated in under 10 minutes. These results showed that the platform was able to support high-volume claims activity while improving the reliability of the wider care delivery process.

By connecting onboarding, agreement management, forecasting and claims generation, providers gained better visibility across service information, funding activity and operational workflows. Teams were able to work with more speed and consistency, reduce manual effort and manage care administration with greater confidence across multiple provider environments.

The result was a stronger, more scalable product platform that reduced operational friction and supported more reliable service delivery. With ongoing enhancement built into the delivery model, the platform could continue evolving with provider needs rather than remaining a one-off implementation.
